The forms on the sites that I work with have been "stripped down" in an effort to get only the contact information that we need, since we also ask additional questions...so in an effort to save space and not overwhelm someone who is filling out our form, we ask for ONE phone number (rather than 3) and an email address, since so many people communicate strictly via email these days.

Hmmmmmm, I think the point on forms should be more about the end user. The form(s) are intended to garner the proper information in order to facilitate an action. The information therein should therefore be direction to exactly what type of data you need to meet this end. In some cases it can be a short and concise form, however, in other it may have to be long and detailed.

SEO should be secondary to the functionality of the forms purpose.

I am always annoyed by superfluous forms...why do you need my home, work and cell number? I'd rather just communicate via email, to be quite honest. Good tips though.

The forms should def. be quick and easy to fill out. People don't want to have to give too much information.
