The form(s) are intended to garner the proper information in order to facilitate an action. The information therein should therefore be direction to exactly what type of data you need to meet this end. In some cases it can be a short and concise form, however, in other it may have to be long and detailed. SEO should be secondary to the functionality of the forms purpose.
